Yes, the elbow points more downward with a more upward swing. That's a result of the arm's anatomy.

But I also observe that even with a more closed paddle face, which means a more forward swing, there is an effort to keep the elbow pointing down. Here are some pics of Samsonov, swinging more forward with a more closed paddle face than Boll in the previous pictures:

And here's one of Maze. Based on the height of the ball, I guess he is hitting a smash, so the swing is at least forward if not also down, yet he still keeps his elbow pointing straight down. Again, there appears to be a lot of tension in the hitting arm.

Yeah, that's how the SH forehand is supposed to be like when you are hitting near your center with a closed paddle. The elbow is more horizontal and natural for the penhold forehand, so you can use your elbow to add more power. Also, you can get a 1-2" inches closer to your center in reach with penhold.

The shakehand forehand is better for drives and smashes though, because the paddle is held higher, while the penhold forehand is better for looping for a variety of reasons.

The shakehands grip naturally supports the bent elbow stroke because the bat freely flexes and your free to stroke however you want whereas with penhold the forwards stroke is better because there are fingers on the back of the blade steadying the face.
